APPLICATION FOR DISCONTINUATION OF SERVICES BY ACCREDITED CERTIFICATION SERVICE PROVIDER

    SUBJECT: APPLICATION FOR DISCONTINUATION OF SERVICES

    The Accredited Certification Service Provider apply for approval of the Council for discontinuation of the Certification Services and for revocation of the Accreditation Certificate, as under:
    2. Date of Accreditation Certificate:
    i) Current validity period:
    ii) Total renewals, if any:

    3. Proof of authority of the applicant to apply for discontinuation:
    i) In case of company: (attach resolution)
    ii) In case of Partnerships: (attach authority letter signed by all partners)

    iii) In case of other bodies: (attach minutes approving the discontinuation with decision of the competent authority)

    4. Requirements:

    i. Contractual arrangements between the parties: (give details of contractual arrangements) (attach certified copy of the agreement)

    ii. Proof of payment of fee: (as per Schedule Il) (attach proof in original)

    5. Brief description of the transferee: (specify the necessary details in respect of the validity and its status and description of Certification Services)


    6. Reasons for transfer: (specify the reasons of transfer and circumstances justifying the transfer of operations of Certification Services)


    7. Request for issuance of no objection certificate: (made a formal request to the Council based on the reasons enumerated in para 6 above)
